Connect Hopes Pty Ltd ABN 68 660 169 522 (Connect Hopes) provides the Connect Hopes online platform for services related to disability appointment booking and management services, provider and participant management services and associated technologies (Platform).

This privacy policy describes how Connect Hopes processes, collects, uses and shares personal data when using this website https://www.connecthopes.com.au (the “website”). Please peruse the information carefully to understand our practices regarding your personal data and how we will process them.

All references to ‘us,’ ‘we,’ and ‘our’ in this Privacy Policy are references to Connect Hopes.
All references to ‘you’ and ‘your’ in this Privacy Policy are references to:

(a) Support Workers, NDIS providers and other service providers for disability/medical services and employees of medical clinics, hospitals or other healthcare providers who are customers, or potential customers, of our products and services (Providers);

(b) the participants or service recipients who themselves, or a person who has authority on behalf of the patient use our Platform, and any other individuals who use our Platform or website to connect with a provider (Participants); and

(c) our contractors and suppliers, potential employees, and any other individuals we might deal with in the course of running our business or providing our services.

We are committed to protecting your privacy and ensuring that the ways in which we deal with your personal information comply with the Australian Privacy Principles contained in the Privacy Act 1988 (Cth) (Privacy Act) and any other applicable health records legislation.

We may update this Privacy Policy periodically and without notice to you. You should review this Privacy Policy from time to time to review any changes. If you have any questions about this Privacy Policy, please contact us using our contact details in section 10 below.

The types of personal information we collect

What is personal data?

We collect information about you in various forms, including personal data. As used in this Policy, “personal information” is as defined in the General Data Protection Regulation; this includes any information which, either alone or in combination with other information we process about you, identifies you as an individual, including, for example, your name, postal address, email address and telephone number.

We collect information about you in the following ways: Information You Give Us. This includes:

  • the personal information you provide when you register to use our website, including your name, date of birth, postal address, email address, telephone number, username, password and demographic information;
  • health and disability-related information or other information which falls within the scope of ‘health information as that term is defined in the Privacy Act;
  • Other ‘sensitive information as that term is defined in the Privacy Act (excluding health information, addressed above), such as information which may relate among other things, your racial or ethnic origin, sexual orientation, criminal record or religious or philosophical beliefs;
  • the personal information that may be contained in any video, comment or other submission you upload or post to the website;
  • the personal information you provide when you report a problem with our website or when we provide you with customer support;
  • the personal information you provide when you make a purchase thorough our website such as bank or credit card details used to transact with us or other financial information that allows us to transact with you or provide you with our services. These will be held by a secure payment provider to ensure they are kept safe. We don’t see your full credit card number, nor do we store it;
  • the personal information you provide when you correspond with us by phone, email or otherwise; and
  • information we may collect from you as a provider, such as your professional qualifications or skills, details of your employment or business history (including details of any personal or professional references and reviews provided to us by third parties);

Information from Social Networking Websites.

Our website includes interfaces that allow you to connect with social networking sites (each a “SNS”). If you link to a SNS through our website, you authorize us to access, use and store the information you agreed the SNS could provide based on your settings on that SNS. We will access, use and store that information in accordance with this Policy. You can revoke our access to the information you provide in this way at any time by amending the appropriate settings from within your account settings on the applicable SNS.

When you access our website, we automatically log information about you and your computer or mobile device. For example, when visiting our website, we log your computer or mobile device operating system name and version, manufacturer and model, browser type, browser language, screen resolution, the website you visited before browsing our website, pages you viewed, how long you spent on a page, access times and information about your use of and actions on our website.     • Our external professional advisers (such as legal advisors); and

They provide services such as data analysis, payment processing, accounts and adminstration, information technology and related infrastructure provision, customer service, email delivery, auditing and other similar services.

User Generated Content

You may share personal data with us when you submit user-generated content to our website, including via our forums, message boards and Websites on our website. Please note that any information you post or disclose on our website will become public information and will be available to other users and the general public. We urge you to be careful when disclosing your personal data, or any additional information, on our website. Personal data and other information will not be private or confidential once published on our website.
